Usability Lab - Tester Waiver

By agreeing to this waiver the I hereby grant full permission to Pixel Tours to take notes of comments made during the usability test and to take video and audio captures of the usability test session.

By agreeing to this waiver the I acknowledge the following,

1. I understand that Pixel Tours employees will review the captured notes, video and audio with the company commishioning the test.
2. I understand that I may be quoted directly in Pixel Tours documents, without reference to my name.
3. I understand that my demographic information age, gender, region may be used in Pixel Tours documents and shared with the company commishioning the test.
4. I understand that my name, street address, phone number and e-mail address will not be shared with external third parties in compliance with the Pixel Tours Privacy Policy.
5. I understand that any material or websites that are presented to me are confidential and as such I may not discuss the content or my opinions with any persons outside of the Pixel Tours employee conducting the test.

This consent and waiver will not be made the basis of a future claim of any kind against Pixel Tours and/or any of its affiliates.
